Online Diabetes Wellness Program

The Online Diabetes Wellness Program (ODWP) is a website that offers convenient access to the information, tools, and support you need to improve your health and diabetes management.

The ODWP helps you with better self-care and enables our Diabetes Education Centre (DEC) staff to assist you remotely and in clinic.

The website is easy-to-use for computer novices of any age, and includes video help guides.

Helpful Features of the ODWP

Learn. A library of resources will help you to improve your knowledge and skills.

Plan. Keep track of your goals, as well as your diet, exercise, and medication schedules. Receive reminders of your required care appointments too.

Record. Input your readings such as blood glucose (automatically!), blood pressure, and weight. The ODWP also provides you with a general Personal Health Record.

Assess. Graphs of your data and wellness surveys help you and our staff to determine how you are doing and provide early-assistance when needed.

Communicate. A secure messaging system connects you and our staff, wherever you are. Use the community forum to find and give support with other people like you living with diabetes.

WHO CAN ENROLL

• DEC clients who have diabetes or who are at risk for diabetes and have access to any computer with an Internet connection
• The Kapuskasing DEC accepts referrals by a healthcare professional
